| Dear Members of the Congregation and Friends, The Session recently discerned that several issues have surfaced in our Session and in the congregation that could be helped by securing the services of a well-trained congregational assessment facilitator. The Stated Clerk of the Presbytery of New York City then contacted the Reverend Dr. David Lee Jones to conduct such a congregational assessment to help us identify any issues through congregational and Session interviews, a Teaching Event, and the adoption of an Action Planning Team who will make recommendations to the Session based on Rev. Jones' report. On Saturday February 26th, Rev. Jones will conduct 15 minute in-person interviews with 20 members of the congregation from 9am-4pm and 6pm-8:30pm. Thereafter, Rev. Jones will be available via email or telephone until March 11th (preferably via email due to time constraints). If you would like to participate in the interviews, please contact the church office. Everyone who participates is asked to complete this consent form which permits Rev. Jones to incorporate comments into his report without identifying anyone by name. We are also asking you to complete these two congregational assessment tools and return them on Sunday February 27th, the day of the Teaching Event, which will be held after the worship service (a light meal will be provided). Steinke's Healthy Congregational Functioning Grid Please use the third page of Steinke's Healthy Congregational Functioning Grid to rank the congregation's functioning on a 1 to 5 scale, with the number 1 being low functioning and the number 5 being high functioning. 15 Anxiety Triggers Survey Every congregation has anxiety - some possess more than others. Please review the list of 15 Anxiety Triggers and rank them according to your personal sense of anxiety, and then your sense of the congregation's anxiety. The number 1 means low anxiety and number 5 means very high anxiety. |
